Residential Critical Care Program
The Residential Critical Care Program is available to qualified critical care customers. The Residential Critical Care Eligibility Determination Form must be completed and returned to your transmission and distribution utility (TDU) before your application for critical care status can be processed.
Please follow the instructions for submission as described on the form. The completed form must be either faxed or emailed by your physician directly to your transmission and distribution utility (TDU) as described by the instructions on the form. The request cannot be processed unless each line of the form is completed by you and the physician. A separate letter from the physician will not be accepted. This program does not waive responsibility for payment of your account.
